These services are designed to ease the consequences of the conditions that would otherwise require the person to move to a higher level of care outside their home.

Services are focused to promote safety, health, well-being, personal growth and independence through empowering individuals and their caregivers. Enhancing the quality of the person's life is their mission.

Southwestern Senior Care, LLC is a PDA Waiver & Options Program provider Certified by the Pennsylvania Department of Aging/Allegheny County Area on Aging.

Southwestern Senior Care services include:

• COMPANION SERVICES... Companion services are provided in accordance with a therapeutic goal in the care plan. The service includes:
  • non-medical care;
  • supervision and socialization to a functionally impaired adult;
  • light housekeeping duties.
  
  
• PERSONAL ASSISTANCE SERVICE... We utilize the "agency directed model", personal assistance provided in a consumer's home that includes:
  • home personal care;
  • other approved activities to consumers with functional disabilities, who need assistance to accomplish daily living tasks in order to live in the community.
  
  
• PERSONAL CARE SERVICES... These services include assistance with:
  • eating, bathing & dressing;
  • personal hygiene and other activities of daily living;
  • according to the plan of care, the service may also include housekeeping chores that are essential to the health and welfare of the consumer.
  
  
• RESPITE CARE... These services are delivered in the home and are designed to:
  • relieve family members or primary caregivers who normally provide care;
  • are short-term in nature.
  
  
• COUNSELING... We provide mental health counseling services to the consumer. Services may address such issues as:
  • problem solving and coping skills;
  • individual/marital and/or family stress;
  • substance abuse;
  • treatment of depression;
  • nutritional education/counseling;
  • family problems involving abuse or neglect.
